DEPARTMENT: Mental Health, Community Therapeutic Services

SUMMARY OF POSITION:

The Therapist, as the Coordinator of Care, is in charge of the client’s treatment during the course of their participation in the program.  The Therapist takes the lead in facilitating treatment services to the client and their family. Therapist performs the intake, completes a thorough assessment of case, develops a Client Treatment Plan (CTP), and provides individual and family therapy based on therapeutic goals. Therapist also coordinates adjunct services, both internal and in the community, to compliment therapy for overall development of client and family. Therapist monitors progress toward all goals and makes adjustment to services and goals as needed. Therapist collaborates with treatment team to determine termination of services and discharge plan.

JOB RESPONSIBILITIES:

Administrative

  1. Adhere to company policies and procedures to ensure legal, ethical and safety standards are maintained.
  1. Comply with all regulatory requirements for professional practice and documentation.
  1. Collaborate with treatment team and applicable outside agencies to promote progress toward treatment goals and family needs.
  1. Ensure that individual billing and productivity objectives are met on a weekly basis.
  1. Complete all documentation requirements on a daily, weekly, and monthly basis—as applicable.
  1. Attend staff meetings and training sessions to enhance knowledge and skills relevant to position and to comply with company policies and state regulations.
  1. Therapist will collaborate with interdisciplinary team to promote and protect client and family’s rights and safety.
  1. Therapist will support mandated reporting laws through clear communication
    regarding state and federal laws, education on developmental stages, and teaching appropriate disciplinary interventions.
  1. Therapist will model and encourage proactive self-advocacy through use of links to resources applicable to ongoing family needs and assertive communication.

CLINICAL

  1. Perform intake, complete a thorough assessment of client, develop a Client Treatment Plan (CTP), and determine appropriate adjunct services and resources to improve the daily functioning of client and family.
  1. Provide therapeutic and behavioral interventions, psychotherapeutic education, and resource linkage to assist clients and families with case management needs and/or therapeutic goals.
  1. Provide individual, family, and/or group therapy to client and family members.
  1. Meet with client, family, treatment team, school staff, and others involved in client’s care to evaluate ongoing needs and services for completion of annual assessments and revised treatment plans.
  1. Deliver timely entries into client charts including (but not limited to) pertinent assessment and intake information, treatment goals and progress, documentation regarding supplemental services, and all progress notes.
  1. Receives training, utilizes, and completes required certification at least one Evidenced Based Practice, within the required time-frame, plus outcome measure data collection and date reporting
  1. Participate in individual and group supervision sessions.
  1. Consistently intervenes by telephone or in person with the client in an emergency crisis to provide support and assistance in problem resolution.
  1. Participate in outreach engagement as well as 24/7 crisis on-call rotation.
  1. Collaborate with family and treatment team to determine progress toward goals and readiness for discharge from program.
  1. Consult with treatment team regarding client and family’s accomplishments, as well as ongoing needs, to insure development of appropriate discharge and referral plan.
  1. Coordinate discharge date with treatment team and family.
  1. Provide termination session(s) to review and reinforce treatment interventions and resources for future need for services.
  1. Participate in new employee orientation and training, specific to therapist duties and responsibilities.
  1. Attend and participate in facility and community trainings to enhance professional development.
  1. Engage with colleagues and supervisors in team building activities to develop and build personal and professional skills relevant to job duties and requirements.

Maintains 65% billable service delivery (26/wk)

Other duties as assigned.

JOB QUALIFICATION STANDARDS

Experience

  • One year direct service experience treating emotionally disturbed children and transitional-aged youth.

 

Education, Professional Training, Technical Training or Certification

  • The position requires a Master’s degree in Marriage and Family Therapy or Social Work, plus registration as a Marriage and Family Intern or Licensed, Associate or Licensed Social Worker, Professional Clinical Counselor Intern or Licensed with the California Board of Behavioral Sciences. Also doctorate in Clinical Psychology with DMH waiver as Registered Psychologist qualifies.  . 
  • Clinical supervisors providing supervision need to have 2 years post license and proof of CE Supervision course for a minimum of 6 hours every 2 years for LMFT, and an additional 1 time course of 15 hours for LCSWs.  
  • It is the employees responsibility to keep license active. Employment is contingent upon active license and in good standing.
  • Therapist must hold a valid driver’s license, maintain current car insurance, and have the ability to travel in the field.

Knowledge/Skills  

  • This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of clients when responding to their clinical needs.
