Presbyterian Heart Group is a truly integrated cardiovascular service line with highly skilled specialists serving multiple hospitals and clinic locations throughout New Mexico. Our group has been awarded IBM Watson top 50 cardiovascular hospital programs two years in a row.

The ideal candidate will be a certified Nurse Practitioner or Physician Assistant with at least two years of experience in Cardiology practice including CHF experience and will enjoy working with peers interested in using the most advanced knowledge in the care of their patients. While the work is primarily clinic based, you will also have the opportunity to care for hospitalized patients as well as provide community outreach and education as needed to expand our programs.

Responsibilities

In the office, you will see patients independently with a cardiologist available for questions when needed. You will integrate the patient’s history, physical examination findings, laboratory results, and imaging results. You will be expected to maintain and elevate the standards of the Heart Failure service and adhere to the Cardiology/Heart Failure Medical Care expectations. You will make referrals for device and advanced therapies when appropriate.

In the hospital, you will sometimes see heart failure patients with the cardiologist, and sometimes independently, depending on the acuity of the patient’s illness. You will be expected to integrate the history, physical examination, laboratory findings, and imaging findings to make decisions about management.

In addition to CHF responsibilities, all of our APCs participate in a weekend call rotation for the cardiology inpatient service.

In all settings, you will be expected to provide timely and complete documentation in the medical record. Professional and concise communication with colleagues, patients and families is essential.

Supportive and collegial relationships with the entire team of caregivers and support staff are crucial to creating an excellent working environment.
