Position Summary:

The Substance Abuse Counselor assesses the patient, formulates a treatment plan and arranges aftercare plan. He/She will also facilitate patient groups, conduct family meetings and handle crisis situations.

Key Responsibilities:

The Substance Abuse Counselor will:
ò Create an initial, multidisciplinary, individualized treatment plan for each patient
ò Conduct initial counseling psych/social evaluations including strengths, needs, abilities, preference for aftercare, addiction and development and psychiatric history and assessment
ò Conduct daily individual counseling session with assigned case load (up to 12 patients) focusing on progress in treatment, needs and aftercare plans, arrangements with community based resources, legal, family, and discharge arrangements
ò Conduct family discharge session
ò Identify resources and refer patient to community service provider, PCP, on-going substance abuse detox or maintenance, and other sources of support and education
ò Lead large group counseling sessions (20-30 patients) utilizing various counseling approaches and formats (didactic, interactive, engagement worksheets) on addiction topics such as relapse prevention, self-help, triggers, feelings, denial, and stress management
ò Identify and complete all necessary releases to complete aftercare arrangements
ò Develop a self-help plan for each patient including education and identification of local meetings

Required Knowledge & Skills:

ò Good communication and interpersonal skills
ò Ability to create a friendly, caring environment for patients, families, visitors, and staff
ò Displays willingness and flexibility in learning new functions

Education:

ò Bachelor's degree in human services, counseling, psychology, or related field. Master's degree preferred

Experience:

ò At least two years experience in substance abuse counseling role, preferably in-patient

Certification/Licensure:

ò CADAC and/or LADAC strongly preferred

Software/Hardware:

ò MS Office, Meditech
